Dr. Michael Puckett
Dr. James M. Puckett is a dedicated and highly experienced dentist serving the Jamestown community from his practice on West Main Street. Since earning his degree from the University of North Carolina in 1993, he has been committed to comprehensive, patient-focused care with an emphasis on prevention and long-term oral health. Known for his compassionate approach, Dr. Puckett and his team prioritize comfort and individualized treatment to ensure every visit is a positive, trusted experience.

Dr. Sidney Chadwick
Background and Education
Dr. Sidney Chadwick was born and raised on the coast in Beaufort, North Carolina. With a long line of Pirates in her family, she followed in their footsteps and attended East Carolina University, where she met her husband, Brandon. She continued her education at ECU’s School of Dental Medicine, earning her Doctor of Dental Medicine degree.
Following dental school, Dr. Chadwick moved to Fayetteville, North Carolina, where she completed an Advanced Education in General Dentistry residency at the Fayetteville VA Medical Center. During her residency, she received advanced training in restorative dentistry, removable prosthodontics including dentures and partials, oral surgery, implant dentistry, and the treatment of medically complex patients.
Dr. Chadwick and her husband have called the Triad home since 2021, and she is proud to care for patients in the community where they are raising their family.
Professional Affiliations
Dr. Chadwick is an active member of the American Dental Association, Academy of General Dentistry, North Carolina Dental Society, and American Academy of Implant Dentistry. She is also a Molis Coaching alumna, having completed advanced training focused on Invisalign treatment.
Outside of the Office
Outside of dentistry, Dr. Chadwick’s favorite role is being a mom to her daughter, Lydia. She enjoys spending time with Lydia and Brandon, along with their adopted fur babies, Orville and Fallon. Having grown up on the North Carolina coast, she still loves boat rides at sunset, pimento cheese and crackers on the beach, riding her bike, reading, and spending time with family and friends. If she could choose another career outside of dentistry, she would happily spend her days running an animal rescue.
Dr. Chadwick strives to make every patient feel heard, comfortable, and confident in their care. Her goal is to build lasting relationships with her patients by listening closely to their needs and helping improve not only their oral health, but their overall quality of life and well-being.
